You can unlock a sixth accessory slot in Terraria by defeating the Wall of Flesh in an Expert Mode world and using the Demon Heart from its Treasure Bag.
The extra slot gives you room for another accessory, which can make a major difference once you enter Hardmode.
Requirements for the Sixth Accessory Slot
Before starting, make sure you have:
- A character that is not using Journey Mode
- An Expert Mode world
- The Guide NPC alive
- A Guide Voodoo Doll
- Enough equipment to defeat the Wall of Flesh
The extra accessory slot will not unlock through this method in a normal Classic Mode world.
Step 1: Create or Enter an Expert Mode World
The important requirement is the world difficulty.
You need to play in an Expert Mode world.
Your character can be taken into that Expert world specifically to obtain the Demon Heart.
Step 2: Reach the Underworld
Travel deep underground until you reach the Underworld.
This is where you’ll summon the Wall of Flesh.
It’s a good idea to prepare a long path through the Underworld before starting the fight so you have enough room to keep moving.
Step 3: Get a Guide Voodoo Doll
You need a Guide Voodoo Doll to summon the Wall of Flesh.
Make sure your Guide NPC is alive before using it.
If the Guide isn’t alive, throwing the doll into lava will not properly summon the Wall of Flesh.
Step 4: Throw the Guide Voodoo Doll Into Lava
Once you’re ready for the fight, throw the Guide Voodoo Doll into lava in the Underworld.
This summons the Wall of Flesh.
Defeat the boss and collect its loot.
Step 5: Open the Wall of Flesh Treasure Bag
Because you’re playing in Expert Mode, the Wall of Flesh drops a Treasure Bag.
Pick it up and open it.
On PC, you can right-click the Treasure Bag from your inventory.
Among the items inside will be the Demon Heart.
Step 6: Use the Demon Heart
Place the Demon Heart in your inventory and use it.
Your character will permanently gain an additional accessory slot.
You can now equip a sixth accessory while playing in Expert Mode.

Why Isn’t My Sixth Accessory Slot Showing?
If the extra slot isn’t appearing, check the following.
You’re Not in an Expert Mode World
The Demon Heart’s additional accessory slot is tied to Expert Mode gameplay.
If you return to a lower-difficulty world, the additional slot won’t function there.
The Guide Was Dead
The Guide must be alive when you throw the Guide Voodoo Doll into lava.
If necessary, wait for him to respawn before attempting the summon again.
You Haven’t Used the Demon Heart
Simply owning the Demon Heart isn’t enough.
You must actually use the item from your inventory to unlock the additional slot.
Can You Get More Than Six Accessory Slots?
The Demon Heart gives you one additional slot beyond the standard accessory setup used in Expert Mode.
It only needs to be used once per character.
Using additional Demon Hearts will not keep adding more accessory slots.
