Every achievement is cumulative, meaning you can chip away at them over time while playing naturally—or strategically farm them if you’re gunning for 100% completion.

Understanding the System: Badges, Not Trophies

Outlast Trials’ achievements function as a series of badges—27 or 28 in total, depending on your platform—that track your behavior, decisions, and repetitions across trials. There’s no hidden trickery; the game tracks your progress cleanly, and you can monitor everything through the in-game menu.

Once you’ve completed the tutorial, your focus will shift to two main areas: prescriptions and rigs. Prescriptions are essentially perks or abilities, while rigs function like deployable tools. Each plays a part in helping you progress and, crucially, in unlocking some achievements.

The slide ability, found under prescriptions, deserves special mention: it’s tied to one of the badges, making it a priority to unlock. As for rigs, there’s no need to commit to one type—achievements are tied to usage, not specific rig classes. You’ll find cumulative counters for deploying, recharging, and making the most of them.

  1. Big Saver: Collect documents. Look for blue folders tagged “confidential.” They spawn randomly but are visually distinct.

  3. Mechanophilia: Use rigs 100 times. With regular play, this should unlock naturally.
  4. Together We Are Strong: Perform 50 co-op actions. Think dual-pushes or team-assisted breaks. Matchmaking helps, but be prepared for randomness in team quality.
  5. Crowd Control: Kick 50 enemies off your co-op partner. Look for chances to save them mid-grab or distraction.
  6. Hyper Condra: Use 200 medical items. Vials come in two sizes—medium and large.
  7. S-Bones: Revive downed co-op partners. Only full revives seem to count.
  8. Speed Freak: Use 50 adrenaline syringes. These are usable any time, no health restrictions.
  9. Breathe In: Use 100 antidotes to recover psych levels (green bar).
  10. Power Surge: Use 100 batteries for night vision. You’ll burn through them quickly, so this comes easily.

  19. Tap Lucidum: Stay in full psychosis for an hour total.

Farming Strategies and Efficiency Tips

Let’s talk farming. For some of the simpler cumulative achievements like “Slip and Slider,” the slide ability is key. You don’t even need to finish a trial for it to count—start a trial, perform the needed action, then exit. It will still track your progress.

The knockdown badge is particularly easy to farm. Inmates can knock you down from hiding spots; find one who repeatedly seeks out the same hiding area and let them topple you over and over. You could earn the entire achievement in under an hour with persistence.

Throwables? Stick with bricks. Unlike bottles, bricks can often be reused. They also count toward both impact and distraction-based achievements.

For health loss, just take damage. Any enemy will do, but the trick is healing effectively afterward. Use the health rig (a healing cloud emitter) to recover in between damage.

If you’re after psychosis-related badges, look no further than the Orphanage mission. One enemy there emits psychosis gas—just stand in the fog, let yourself go mad, then recover and repeat. Simple, if not exactly pleasant.

Choosing the Right Trials

Each of the four core programs (trial groups) features unique environments and item placements. The objectives stay consistent, but item locations, trap placements, and even doors will randomize slightly every time. Still, once you learn a map, you’ll get a feel for efficient routes.
